Summing it Up
Transition planning is looking at all areas of a student's life, not just what he/she is doing in school. The family, adult service providers, and the schools are there to help the individual succeed in all aspects of life. The goal is to create a plan that will move past survival to successful independence. This plan needs to be a guiding factor for their daily education programming. We all need to work together to insure that the transition plan becomes a functionally active program for the individual, and that it is addressed and updated annually.
