New Horizons: Guide to Student Transition

What are the areas of transition?
As mentioned on page one, there are four areas of transition:

  1. Instruction/Related Services - Examples: classroom instruction, portfolio development.
  2. Community Involvement - Examples: recreation and leisure activities, development of personal/social skills, mobility/transportation training.
  3. Development of Employment - Examples:  job shadowing, further education, vocational training, work-study/co-op participation.
  4. Daily Living Skills - Examples: self care, home management, finance and budgeting, training, medical concerns.

At age 14, transition must be addressed at the IEP.  The purpose is to focus attention on how the child's educational program can be planned.  At age 16, a transition plan must be written that addresses each of the four areas of transition.
